Use this form to report missing items, missing packages, wrong styles, wrong sizes, wrong quantities, damaged items, or any other order issue.

Before submitting Please double-check your invoice, packing list, boxes, and style labels before submitting. If multiple employees handled the shipment, please confirm with your team first to make sure the items are not misplaced, already stored, or separated from the original package.

Include all affected styles in one report when possible.
